Dog Microchipping Form

Since the 6th April 2016, it has been compulsory for all dogs in England to be microchipped whether they have a tag or not.
Dog owners who do not comply with the micochipping law could face fines of up to £500.
Northumberland County Council Animal Welfare team offer a dog microchipping service.
An officer will visit your home and microchip your dog at a cost of £10 per dog.
This service is only available to residents of Northumberland and dogs must be at least 8 weeks old.
